优质护理模式在局部麻醉下经皮肾镜碎石术中的应用

| 摘 要： | 目的: 探讨优质护理护理管理模式在局部麻醉下经皮肾镜碎石取石手术患者中的应用体会。方法: 将2020年9月至2022年9月收治的80例局麻经皮肾镜碎石患者采取优质护理模式（护理模式的信息化、护理模式的个性化、护理模式的全程化），术前通过影像、图片、微信、宣传册等做好综合评估、心理护理、手术方式宣教、体位训练，术中做好体位的摆放、保温、心理指导，术后注意引流管的管理、疼痛的监护、出院指导和随访。比较患者术前术后心理状态、疼痛评分、护理满意度。结果: 90例患者均手术成功，3例出现胸腔积液，4例出现引流管的堵塞，2例出现体温的上升，3例出现明显血尿，均予对症治疗及护理后恢复。患者术后心理状态、疼痛评分、护理满意度明显改善（P < 0.05）。结论: 局麻经皮肾镜碎石取石术具有创伤小、恢复快优势，但患者可能存在紧张、焦虑的不良情绪，通过优质护理管理模式，能有效缓患者不良情绪、改善睡眠质理、减轻疼痛，促进患者的快速康复。 |

Application of High-Quality Nursing Mode in Percutaneous Nephrolithotomy Under Local Anesthesia

| Abstract: | Objective: To explore the application experience of high-quality nursing management mode in patients undergoing percutaneous nephrolithotripsy under local anesthesia. Methods: From September 2020 to September 2022, 80 cases of percutaneous nephrolithotomy under local anesthesia were treated with high-quality nursing mode (informatization, individualization and whole process of nursing mode). Before operation, comprehensive evaluation, psychological nursing, propaganda of operation methods and posture training were done through images, pictures, WeChat and brochures. Posture placement, heat preservation and psychological guidance were done during operation, and attention was paid to drainage tube after operation. The psychological state, pain score and nursing satisfaction of patients before and after operation were compared. Results: All the 90 patients were successfully operated, with pleural effusion in 3 cases, blockage of drainage tube in 4 cases, temperature rise in 2 cases, and hematuria in 3 cases. All patients were cured after symptomatic treatment and nursing. After operation, the psychological state, pain score and nursing satisfaction of the patients were significantly improved (P<0.05). Conclusion: Percutaneous nephrolithotomy under local anesthesia has the advantages of less trauma and quick recovery, but patients may have bad feelings of nervousness and anxiety. High-quality nursing management mode can effectively relieve patients' bad feelings, improve sleep quality, relieve pain and promote patients' quick recovery. |
